令和６年２月２８日、当局発注「八戸港八太郎・河原木地区航路泊地（埋没）浚渫工事」において、排砂管撤去作業の際、北防波堤に仮係留していた排砂管のワイヤーが波浪により破断した。その後排砂管が浮遊し航路側に流出するおそれが生じたため北防波堤上でワイヤーを再固縛していたところ、防波堤を越える波浪により作業員１名が海上に転落、排砂管に激突し死亡した。また、作業船上の作業員６名が急激な動揺により船上で転倒するなど負傷した。 当該事故は、防波堤を越える波浪がみられ、防波堤上での作業に危険が伴う認識はあったものの、緊急性を優先し安全面を軽視したことなど、工事の安全管理を怠ったため発生したものである。

（株）清水電気商会は、令和８年３月３日開札の三重河川国道事務所発注「令和７年度四日市管内照明設備整備工事」において、共通仕様書で定められた品質証明員を選出できないため 、落札決定後に契約を辞退した。

四国地方整備局土佐国道事務所発注の「令和６年度 越知道路越知丁改良第６（その２）工事」において、安全管理措置の不適切により、足場床を組立作業中、下請作業員が未固定の足場板の張出部に乗ったことでバランスを崩し転落し、負傷する事故を発生させた。

令和６年７月25日、(株)佐藤建設が受注し、施工中であった湯沢河川国道事務所発注の「雄勝山道路改良工事」において、当日は連続雨量が中止基準を超過し、休工であったにもかかわらず、作業員が現場資機材の回収のため現場に入り、引き上げている際に現場内の上部法面が崩落し、土砂に巻き込まれた作業員のうち１名が死亡する事故を起こした。

上記業者が、当局が発注した「令和７年度 徳島運輸支局設備改修工事監理業務」の入札において、調査基準価格を下回る金額で入札を行った。そのため予算決算及び会計令第８６条に基づく調査を通知したところ、業者から、低入札価格調査による関係資料の提出及び事情聴取に協力できない旨、申し出があった。

Daiwa Denki Co., Ltd. participated in the bid for the Kuji Port Tsunami Observation Point Cable Laying Work conducted by this agency, opening the bid on October 15, Reiwa 7, and was awarded the contract. The qualified contractor was the successful bidder for the 'R5/6 National Route 289 Construction Road Measures External Works' ordered by the Nagaoka National Highway Office of the Hokuriku Regional Development Bureau, with the bid opening on February 26, Reiwa 6 (2024). The contract was signed on March 6. However, on March 26, the contracting authority canceled the contract because the contractor could not assign the planned technical personnel to the project.

In the 'Reiwa 4th Year Hijikawa Bridge Replacement Detour Temporary Bridge Removal Work' ordered by the Daisou River and National Highway Office of the Shikoku Regional Development Bureau, an employee of Murakami Kogyo Co., Ltd., the contractor, removed some steel materials generated during the removal of the temporary bridge and sold them privately. As a result, the contractor was unable to deliver the materials to the client by the completion deadline, leading to partial non-fulfillment of the work.

Kurisuki Construction Industry Co., Ltd. concluded a contract on October 31, 2023, with the Otagawa River Office for the 'Reiwa 5th Year Misasho River Karyuike District Embankment Maintenance and Other Works.' However, the company did not commence work on site and submitted a notice of impossibility of performance on April 16, 2024. The Otagawa River Office determined that this situation fell under Article 48, Paragraph 1, Item 10 of the Construction Contract, and accordingly, canceled the contract on April 22, 2024.

However, the company mistakenly submitted a bid lower than the planned amount and submitted a contract withdrawal notice on April 18, 2024.

The contractor caused a work-related accident resulting in a fatality during high tree felling operations in the R5 Iruma River maintenance work ordered by the Arakawa Upper Stream River Office on February 9, Reiwa 6 (2024), around 14:19. After completing the face cut and during the back cut, signs of tree movement appeared. A worker evacuated to the back cut side and contacted a vertically split remaining tree, resulting in death.
