In the construction of the P9 substructure of the Tsuda Elevated Bridge on the transverse road, contracted by the Tokushima River and National Highway Office of the Shikoku Regional Development Bureau, Inoue Corporation, a qualified contractor, used a pre-installed scaffolding for rebar assembly during the construction of the bridge pier. When the inclination of the rebar and scaffolding was confirmed and corrections were made, 142 vertical reinforcement bars (D51) had already been installed and collapsed, resulting in the death of one worker and injury to three others on the scaffolding. The accident occurred during the assembly of the reinforcement bars.

上記有資格業者である(株)井上組は、四国地方整備局徳島河川国道事務所発注の「令和３年度横断道津田高架橋下部Ｐ９工事」において、橋脚躯体の施工にあたり、先行して設置した鉄筋組み立て用の架台を使用しての柱部鉄筋Ｄ５１の組み立て（建込み）作業中、鉄筋及び架台の傾きが確認されたため、建てりを修正していたところ、既に建込んでいた１４２本の柱部鉄筋Ｄ５１が倒壊して、架台上の足場に乗っていた一次下請の作業員１名が死亡し、３名が負傷する事故を発生させた。
