上記有資格業者である(株)井上組は、四国地方整備局徳島河川国道事務所発注の「令和３年度横断道津田高架橋下部Ｐ９工事」において、橋脚躯体の施工にあたり、先行して設置した鉄筋組み立て用の架台を使用しての柱部鉄筋Ｄ５１の組み立て（建込み）作業中、鉄筋及び架台の傾きが確認されたため、建てりを修正していたところ、既に建込んでいた１４２本の柱部鉄筋Ｄ５１が倒壊して、架台上の足場に乗っていた一次下請の作業員１名が死亡し、３名が負傷する事故を発生させた。
