株式会社西山工務店は、同社が豊岡河川国道事務所から請け負った兵庫県豊岡市内の向日置川道路改良工事において、Ｕ型擁壁の施工時、発注者と協議を経ずに隣接する施工済みＬ型擁壁下部の地盤改良体を一部撤去し、強度確認を行わずに土砂で埋め戻しを行ったところ、Ｌ型擁壁の沈下・傾きが発生した。 また、沈下・傾きを隠すために、同社にてモルタルによる上塗り補修を行ったため、その補修箇所に変位によるクラックが発生し、発注者が発見したものである。
