令和４年１２月２０日、１２月２１日及び令和５年１月１８日に、貴社に対して保安監査を実施したところであるが、保安監査の結果、改善を要する事項が認められたことから、下記の事項について、改善措置を講ずるよう指示する。講じた措置については、令和５年６月８日までに報告されたい。なお、改善措置を講ずるにあたっては、背後要因を含め当該事象が発生した原因を究明したうえで、再発防止に必要な改善策を策定するとともに、輸送の安全に係る業務が確実に実施できるよう留意すること。記１．軌道の検査について、以下の事実を確認した。（１）軌道・土木設備実施基準第１３６条及び第１３７条に規定している一般軌道（本線、側線）の直線区間の軌道狂い検査、並びに一般軌道（本線及び側線）の曲線区間及び分岐器（本線、側線）における通り及び高低の軌道狂い検査を実施していなかった。また、同軌道狂い検査において、整備基準値に達している箇所が複数確認されたにもかかわらず、必要な整備を行っていなかった。（２）一部の分岐器について、軌道・土木設備実施基準第１４０条第１項及び第２項に規定する分岐器の損傷、減耗及び腐食等の状態、並びに密着、接着、バックゲージ及びその他重要部分の付属品の状態について検査を実施していないことを確認した。（３）軌道・土木設備実施基準第１３８条に規定するレールの遊間検査の結果に基づく整備を実施しておらず、適切な管理がなされていないことを確認した。また、令和元年度及び２年度のレールの遊間検査を実施していないことを確認した。（４）軌道・土木設備実施基準第１０８条に規定する橋りょう及び土留壁の検査について、実施していないことを確認した。よって、速やかに検査結果に応じた必要な整備を行うとともに、このような事象が二度と発生しないよう徹底した原因究明を行うこと。また、法令等の遵守を含め実施基準等に基づく保守管理が適切に実施されるよう関係者に徹底するとともに、社内における教育内容の見直し及び再教育、並びに検査及び整備実施状況に係るチェック体制の強化を図る等、管理方法及び体制を見直すこと。さらに、講じた措置が継続的に実施される仕組みを構築すること。２．電気設備実施基準第８０条及び同実施基準別表1「電気設備検査方法」に規定する電気転てつ機の定期検査について、陸前山王駅構内の電気転てつ機は、他の鉄道事業者に測定業務を委託しているが、当該検査結果の記録を確認せず、結果の判定を実施していないことを確認した。すでに測定結果の記録を確認し、結果に問題はないことを確認したとの報告を受けたところであるが、過去の改善指示に基づき確実に検査を行うとの改善報告があったにもかかわらず、再度不適切な状態に陥っていることから、これを重く受け止め、このような事象が二度と発生しないよう徹底した原因究明を行うこと。また、今後の鉄道施設の維持管理にあたっては、法令等の遵守を含め実施基準等に基づく保守管理が適切に実施されるよう関係者に徹底するとともに、社内における教育内容の見直し及び再教育、並びに検査及び整備実施状況に係るチェック体制の強化を図る等、管理方法及び体制を見直すこと。さらに、講じた措置が継続的に実施される仕組みを構築すること。【東北運輸局】
