（1）平成28年9月14日付けで申請のあった旅行業の更新登録について、申請書類の「選任取扱管理者の履歴書」に、自身の経歴について事実と異なる記載をし、虚偽の申請を行い、同年10月27日からの大阪府知事による更新登録を受けた。（旅行業法第19条第1項第3号に該当） （2）平成28年9月14日付けで申請のあった旅行業の更新登録について、申請書類の「選任取扱管理者の履歴書」及び「選任取扱管理者の欠格事由に該当しない旨の宣誓書」に、東京営業所の選任取扱管理者の住所について事実と異なる記載をさせ、虚偽の申請を行い、同年10月27日からの大阪府知事による更新登録を受けた。（旅行業法第19条第1項第3号に該当） （3）平成23年10月27日付けで登録された主たる営業所について、旅行業務取扱管理者として選任できない者を届出し、又、旅行業務取扱管理者が欠けた状態で旅行業務を行った。（旅行業法第11条の2第1項違反） （4）平成28年7月19日付けで届出のあった東京営業所について、旅行業務取扱管理者として選任できない者を届出し、又、旅行業務取扱管理者が欠けた状態平成28年11月17日まで旅行業務を行った。（旅行業法第11条の2第1項違反） （5）平成23年10月27日に旅行業登録を受けた後、平成27年から現在までの間、自社では旅行業は行わず、旅行業登録の無いクリエイティブジャパン有限会社に旅行業を行わせた。（旅行業法第14条違反） （6）旅行業法第10条の取引額の報告について、平成23年10月27日に旅行業登録を受けてから現在までの間、全て虚偽の報告を行った。（旅行業法第10条違反）
