貴局に対しては、令和３年１０月１３日から１０月２８日まで保安監査を実施した。その結果、以下のとおり改善を要する事項が認められたことから、改善措置を講ずるよう指示する。改善措置を講ずるにあたっては、発生した背後要因を含めて原因を究明した上で、同種事象の再発防止のための措置を講ずること。なお、講じた措置については、令和４年１月末日までに報告されたい。記１．軌道運転規則第４条に基づく、軌道電気施設整備心得について、変電所を新設した際に同規則第４条の２に規定する地方運輸局長への届出を行わないまま、同心得を変更している事実を確認した。よって、速やかに同心得の変更届出を行うとともに、関係法令の理解を深め、同規則に基づく手続きを確実に行うよう管理体制を見直すなど、必要な措置を講ずること。２．軌道運転規則第４条に基づく、線路整備心得第２条に規定する軌道定期検査及び線路施設等の検査について、以下の事実を確認した。 ①信号機の設置されている交差点を除く本線路について、線路施設等の検査のうち「レール締結状態」の検査を実施していないこと。 ②電車事業所構内の一部区間を除く構内線について、軌道定期検査のうち「通り」と「高低」などの検査を実施していないこと。 ③線路整備心得第２条で規定する検査項目のうち、線路施設整備マニュアルでは、側線の軌道定期検査の「通り」と「高低」が規定されていないこと、及び線路施設検査マニュアルでは、交差点を除く本線路の「レール締結状態」の検査が規定されていないこと。よって、検査を実施していない箇所について速やかに必要な検査を実施するとともに、心得とマニュアルが適合するよう関係規程を見直すこと。また、本関係規程以外の規程類も点検して、必要に応じ見直すとともに、安全を確保するために必要な関係規程を遵守する重要性を再認識し、関係規程の適切な管理及び検査が実施できるよう体制を見直すなどの措置を講ずること。３．軌道運転規則第４条に基づく、軌道電気施設整備心得及び電車整備基準の検査について、以下の事実を確認した。これらについては、改めて関係規程の内容を社内に周知徹底し、遵守すること。（１）軌道電気施設整備心得について電気回路の絶縁抵抗の測定結果（令和３年５月１日にすすきの停留場構内で発生した電気火災事故に伴い修理）について、外注先には記録が保存されていたが、貴局には保存されていないこと。（２）電車整備基準について ①８５００形車両の重要部・全般検査に係る総合検査について、「砂まき管の高さ」を規定せず、検査を実施していないこと。 ②Ａ１２００形車両の重要部・全般検査に係る総合検査において、「救助器及び砂まき管の高さ」及び「運転士異常時列車停止装置の機能」の検査を実施していないこと。【北海道運輸局】
