Original Japanese text	令和５年２月２０日から２２日、３月６日及び７日、５月９日から１１日、７月２０日及び２１日に、貴局に対して保安監査を実施したところである。監査の結果、改善を要する事項が認められたことから、下記の事項について、改善措置を講ずるよう指示する。なお、改善措置を講ずるにあたっては、発生した背後要因を含めて原因を究明したうえで、再発防止のための措置を講ずること。また、講じた措置については、令和５年１０月２４日までに報告されたい。記１．鹿児島市交通局電車安全管理規程（以下「安全管理規程」という。）第１３条に規定する事故・災害等の防止対策の検討について、令和５年５月１日に谷山線郡元（南側）停留場～郡元停留場間交差点での運転取扱い誤りによる信号冒進に起因した車両脱線事故を発生させた際に、当該事故を踏まえ、貴局において鹿児島市交通局事故等調査委員会（以下「調査委員会」という。）を開催しているが、再発防止対策として当該運転士への机上教育及び添乗教育並びに全ての職員に注意喚起等を実施しているものの、運転取扱い誤りが起きる様々な要因を貴局全体の問題と捉えずに、現状の運転取扱いや施設の状況などを踏まえた検証を行っていないことを確認した。また、これまで運転士の取扱い誤りによる信号冒進が複数回発生していることに関して、調査委員会を開催しておらず、原因究明及び再発防止対策を講じていないことを確認した。よって、安全管理規程第１３条の規定に基づき、安全統括管理者は、調査委員会の場を用いて、過去に発生した運転士の取扱い誤りによる信号冒進に関する全ての事象について、運転取扱い面に限らず施設の状況の検証を行うなど的確に原因分析を行うとともに、必要な対策等を早急に実施すること。また、事故の発生を未然に防ぐための安全管理体制を適切に機能させるための改善を行うこと。なお、今後、同種事象が複数回発生する場合においても、調査委員会を開催するなど、的確に原因分析し、その再発防止対策を講ずること。２．鹿児島市電気軌道運転取扱心得第７条に規定する係員の教育及び訓練について、以下の事実を確認した。（１）電車運転関係係員教育訓練規程（以下「教育訓練規程」という。）第８条に規定する知識技能の認定において、教育及び訓練を受けた運転士が知識技能を保有することを確かめるために口頭諮問又は面接諮問を行うこととしているが、実際には口頭諮問又は面接諮問とは異なる方法で行っており、また、一部の運転士に対して知識技能を保有することを確かめずに、運転士の作業を行わせていたこと。（２）教育訓練規程に規定する運転士の教育及び訓練について、安全管理規程第８条第７項では、「運転管理者は、運転関係の係員に対する教育・訓練を適切に管理する」と規定しているにもかかわらず、運転管理者は、教育内容、教育方法、進捗管理、知識技能の確認の全ての段階において、教育訓練規程と実態が乖離していることを認知しないまま、同規程に基づく管理ができていない状況であること。よって、運転管理者が運転士に対する教育及び訓練に積極的に関与する体制を構築すること。また、教育及び訓練が適切に行われるよう、教育方法等を検証し必要な見直しを行うこと。３．鹿児島市電気軌道運転取扱心得第７条で規定する適性検査（身体機能検査）のうち視機能（視力）及び聴力について、乗務員１名が合格基準に達していないにもかかわらず動力車を操縦する作業を行わせていたことを確認した。また、「運転に関係のある係員の適性検査等に関する規程」第５条で規定する身体機能検査の合格基準について、職種や検査方法に応じた基準を定めていないことを確認した。よって、身体機能検査の合格基準について職種や検査方法に応じて速やかに定め、乗務員１名に対し速やかに視機能（視力）及び聴力検査を実施し、検査の結果が作業を行うのに支障がないことを確かめるとともに、身体機能検査について、適切な実施と確実な適性の有無の確認が行えるように管理体制の見直しを行うこと。４．軌道事故等報告規則第３条に規定する運転事故等届出書について、令和３年４月３日及び令和４年１２月９日の道路障害事故並びに令和４年６月１０日の輸送障害について、同届出書が提出されていなかったことを確認した。また、軌道法施行規則第３５条の２に規定する異常運転等報告書について、令和３年２月１２日、令和４年２月１３日、同年７月１７日、同年７月２０日及び同年１１月２５日に信号冒進が発生しているにもかかわらず、同報告書が提出されていなかったことを確認した。よって、過去に発生した事故等についても再度確認し、届出が必要なものについては提出を行うこと。今後、両規則に該当する事故等の提出の判断が適確に実施されるよう管理体制を改善すること。以上【九州運輸局】
