貴局に対しては、令和６年１月以降、運転事故、重大インシデントを含む多数の事象発生の都度、原因究明及び再発防止対策の策定を求めてきたところである。 また、「保安監査の結果について」（令和６年９月２０日付け九運鉄監第１９号）においても改善措置を講ずるよう指示したところであり、貴局において改善に向けて取組んでいる最中にもかかわらず、令和７年３月２５日に熊本城・市役所前停留場内の車両衝突事故により多数の負傷者を発生させた。 当該事故の原因については、現在、運輸安全委員会により調査中であるが、この事故を受け、令和７年３月２７日、同年４月１５日から１８日に保安監査を実施した結果、新たに下記のとおり改善を要する事項が認められたことから、改善措置を講ずるよう指示する。 なお、改善にあたっては、「保安監査の結果について」（令和６年９月２０日付け九運鉄監第１９号）を受けて改善を行った事項についても再度検証を行い、必要な措置を講ずること。 また、講じた措置については、令和７年８月１８日までに報告されたい。 記 以下の事項をはじめとして、各管理者が現場の状況を的確に把握する体制を整備した上で、現場の業務の実施状況を定期的に検証して、課題を整理し、必要な改善を行うとともに、改善の実行性が確保されるよう安全管理体制の強化を図ること。 １．軌道運転取扱心得第７条に規定する動力車を操縦する作業を行う係員に対す る教育の実施並びに知識及び技能の保有の確認に関して、令和３年度から令和６年度の記録によると、一部の運転士において軌道運転取扱心得第５６条に規定する追従する場合の運転速度に関する技能の保有の確認を行わないまま動力車を操縦する作業に就かせていたことを確認した。 よって、動力車を操縦する作業を行う係員が、当該作業を行うために必要な知識及び技能の保有の確認の項目及び方法について改めて検証した上で必要な見直しを行うとともに、当該係員に対する教育並びに知識及び技能の保有の確認を適切に実施するための管理体制を構築すること。また、管理体制を構築した際は、当該係員を管理する者を含む関係者に対する教育を実施すること。 ２．軌道運転取扱心得第５２条及び第５３条に規定する車両の放置の禁止等において、車両から離れる際はレバーシングハンドルを携帯することとなっているが、レバーシングハンドルを携帯せず、定められた車両の無断使用防止の措置を適切に実施しないまま車両から離れている運転士がいたことを確認した。 よって、やむを得ない場合に車両を離れる際の現在の手順等の検証を行った上で、各規程等の必要な見直しを行い、必要な措置を確実に行った後に車両を離れるしくみの構築を行うこと。また、しくみを構築した際は、運転士及び当該係員を管理する者に対する教育を実施すること。 ３．軌道運転取扱心得第１５条に規定する運転前に行う車両の点検において、当該点検の結果、車両に異常があった場合は、点検をした者から管理者に報告しているとのことであるが、令和７年１月から３月の記録を確認したところ、次のことを確認した。 （１）点検結果の一部又は全部の記録がないものがあったこと。 （２）点検結果の記録を管理者が確認していないこと。 よって、運転前に行う車両の点検結果の報告手順及び点検結果の記録の確認方法を検証した上で必要な見直しを行い、運行前に行う車両の点検に係る管理体制を構築すること。また、管理体制を構築した際は、運行前の車両の点検結果を管理する者を含む関係者に対する教育を実施すること。 以上 【九州運輸局】
