貴社においては、令和４年２月７日に多賀線高宮駅構内において列車脱線事故（以下「本件事故」という。）を発生させた。原因については、運輸安全委員会で調査中であるが、事故後に貴社からは、レール締結不良による軌間拡大が影響したと推定しているとの報告があった。これを受けて、貴社に対して、令和４年２月２１日及び２２日に保安監査を実施したところ、下記のとおり改善を要する事項が認められたことから、所要の措置を講ずるよう指示する。改善措置を講ずるにあたっては、当該事項に係る業務の実施方法、実施状況、管理方法等の妥当性について検証する等により、背後要因を含め当該事項が発生した原因を究明したうえで、再発防止に必要な改善策を策定するとともに、輸送の安全に係る業務が確実に実施できるよう留意すること。なお、講じた措置については、令和４年５月２３日までに報告されたい。記１．土木施設実施基準第５４条（軌道の検査）で規定する、軌道部材の定期検査の記録と脱線事故後に実施したまくら木の緊急点検の記録を確認したところ、まくら木検査のレール締結状態の良否判定において、定期検査では良判定となっていたが、その後の緊急点検では否判定となっていた箇所が相当数あることを確認した。レール締結装置の検査について、定期検査においては目視で緊急点検においては打音及び触手を追加して実施していた。定期検査マニュアル（軌道編）を確認すると検査項目（着眼点）、検査方法及び判定方法が明確に規定されていなかった。よって、レール締結装置の検査が確実に実施できるように定期検査マニュアル（軌道編）で検査項目（着眼点）、検査方法及び判定方法を整備するとともに、関係規程の管理及び関係規程に基づく検査が適切にできるよう必要な措置を講ずること。２．施設の保全を行う係員に対し、レール締結装置の維持管理について、定期検査マニュアル（軌道編）に検査項目（着眼点）、検査方法及び判定方法が明確に規定されておらず、適切に教育が行われていないことを確認した。よって、当該装置の検査を行う係員に検査項目（着眼点）、検査方法や判定方法を正しく理解させるため、教育方法を見直し、適切に教育を行うなど必要な措置を講ずること。また、レール締結装置以外の検査においても、１．及び２．の指示を踏まえ、現状を確認した上で、必要な改善を図ること。【近畿運輸局】
