令和８年６月５日、内部調査により、機関長が受有する海技免状が失効したまま複数回にわたり旅客運送に従事していたことが判明した旨の報告があった。同年６月24日、７月２日及び７月17日、関東運輸局の運航労務監理官が海上運送法に基づく検査を実施したところ、安全管理規程に違反する事実を確認した。同年10月26日、関東運輸局は同者に対して、「船舶借入人は、船舶職員及び小型船舶操縦者法第18条第１項に基づき、運航する船舶に乗組み基準に定める有効な海技免状を受有する海技士を乗り組ませることについて、管理体制の見直しを行い、これを確実に遵守するものとすること。また、運航船舶に乗組む船舶職員は、同法第21条第１項に基づき、乗組み基準において必要とされる資格に係る有効な海技免状を受有すること。」を含む命令を行った。
