1. Failure to appoint a supervising engineer: The operator entered into a subcontract with Keitech Corporation exceeding the statutory amount for work ordered by Osaka City. Despite this, they appointed Mr. A as a dedicated chief engineer at the construction site from the contract date until the day before the qualification announcement, and until the day before the issuance of the supervising engineer qualification certificate, without possessing the required qualifications or holding the certificate, in violation of Article 26, Paragraphs 2 and 5 of the Building Business Act. 2. Failure to create construction management documentation: The operator falsely documented that Keitech Corporation was not directly contracted for the construction work, in violation of Article 24-8, Paragraphs 1 and 4 of the Building Business Act, despite having directly contracted the work to Keitech. 3. Lump-sum subcontracting: The operator violated Article 22, Paragraph 1 of the Building Business Act by failing to properly assign qualified supervising engineers and subcontracted the work entirely to Keitech Corporation without proper oversight.
