Actual conditions regarding regulations: - The pressing work of wheel axles was outsourced without following internal safety management regulations. - The contractor lacked regulations on pressing force values and handling deviations; they verified pressing force by obtaining drawings or calculating values themselves. 2. Actual conditions at the site: - Deviated wheel axles were used without proper approval, and this practice was orally and tacitly continued. - The contractor believed deviations in pressing force were acceptable. 3. Staff knowledge and training: - No systematic education on wheel axle assembly was provided. 4. Record modification: - Records could be and were actually modified at the contractor. - Record modifications were orally and tacitly accepted within the workplace. 5. Management of work: - Supervisors did not verify whether wheel axles could be used. These facts violate the standards set forth in the Ministerial Ordinance No. 87, Article 4, and Article 18-3, Paragraph 2, of the Railway Business Act. Based on these findings, Tokyu is instructed to: (1) Develop internal regulations regarding pressing work. (2) Improve management of staff education and training. (3) Establish mechanisms to prevent record modifications, including verification procedures. (4) Review and enhance safety management systems across departments. Tokyu must submit a plan or take measures for items (1) to (4) by January 31, 2025, and for item (4) by March 31, 2025.
