Abbott Medical Japan LLC has initiated a voluntary recall of the medical devices listed below, as informed by Tokyo Metropolitan Government.

The Tokyo Metropolitan Government has provided information that Cook Medical Japan LLC has initiated a voluntary recall of the medical devices listed in the attached document.

Tokyo Metropolitan Government provided information that Japan Medtronic Co., Ltd. has initiated a voluntary recall of the medical devices listed below.

Denkai Corporation has initiated a voluntary recall of certain lots of its in vitro diagnostic medical device (antigen rapid test kit) following reports from Niigata Prefecture. Initially, a recall was conducted for some lots of products that provide results in 15 minutes starting from November 8. After ongoing investigation, it was found that for some lots of products that provide results in 8 minutes, the false positive rate increased over time. As a result, the recall has been expanded to include additional products.

Niigata Prefecture has provided information that Denka Corporation has initiated a voluntary recall of the following in vitro diagnostic medical device (antigen rapid test kit).

Regarding Moderna's COVID-19 vaccine, multiple reports of foreign matter contamination in a single lot (Lot 3004667) while unused led to a voluntary recall of that lot. Although no reports of contamination were made for other lots, two lots (Lot 3004734 and Lot 3004956) manufactured on the same equipment and at the same time were also voluntarily recalled.

On January 18, 2019, Tokyo Metropolitan Government was informed that Japan Medtronic Co., Ltd. has initiated a voluntary recall of certain medical devices. Additional products and serial numbers have been included in the recall as detailed in the attached information.

Under the guidance of Yamaguchi Prefecture, Imura Sangyo Co., Ltd. is proceeding with a voluntary recall of imported frozen shishamo. Today, Yamaguchi Prefecture announced that during the guidance process, five new types of labels were identified on the subdivided products.
