株式会社土田建設および同社の代表取締役は、同社が施工した越前市発注の東単１２東部６号汚水支線工事において、車両系建設機械を用いて鋼矢板をつり上げ、トラックへ積み込む作業を行うに際し、運転中のドラグショベルに接触することにより労働者に危険が生ずるおそれのある箇所に、労働者を立ち入らせない等機械による危険を防止するための必要な措置を講じなかった。この結果、令和２年１１月４日、鋼矢板の積み込み位置の調整していた労働者にドラグショベルが接触し、当該労働者は左足腓骨脛骨を骨折し、処置の結果、左下肢高位を切断した。 このことについて、同社および同社の代表取締役が、武生簡易裁判所から労働安全衛生法違反により、それぞれ罰金刑を受け、令和３年１０月２０日にその刑が確定した。
