Tsuchida Construction Co., Ltd. and its representative director used construction machinery to lift and load steel sheet piles onto trucks during work ordered by Echizen City. They did not take necessary measures to prevent machinery-related hazards, such as restricting worker access to areas where contact with operating drag buckets could occur. On November 4, 2020, a drag bucket contacted a worker adjusting the loading position, causing a fracture to the worker's left fibula and tibia, which led to the amputation of the worker's left lower limb. The company and its representative were each fined for violations of the Industrial Safety and Health Act by Takefu Summary Court, with the penalties finalized on October 20, 2021.
