令和５年４月１１日に長尾線花園駅から林道駅間の上福岡踏切道において、踏切遮断機が遮断していない状態で列車が踏切内に進入し、停止した事象を発生させた。貴社においては、令和３年１月２８日、同年１２月３日、令和４年２月８日及び同年４月１４日にも同種事象を発生させ、再発防止対策を進めていたにもかかわらず、今般、本事象を発生させた。このことを踏まえて、貴社に対して、令和５年４月１３日、１４日及び２６日に保安監査を実施した。その結果、下記のとおり改善を要する事項が認められたことから、所要の措置を講ずるよう指示する。改善措置を講ずるにあたっては、今回の事象が発生した背後要因を含め詳細に調査し、原因を究明したうえで、同種事象の再発防止に必要な改善策を策定するとともに、輸送の安全に係る業務が確実に実施できるよう留意すること。なお、講じた措置については、令和５年７月３１日までに報告されたい。記１．本事象の原因は、踏切制御用の電源を供給している柱上変圧器の２次側ヒューズが破断したため、踏切遮断機が遮断しなかったとの報告があった。電気設備実施基準第６７条に規定する電気設備の定期検査の実施状況等について、以下の事実を確認した。（１）令和３年度のヒューズの検査結果記録を確認したところ、すべて「良」との記載であった。しかしながら、本事象の緊急の再発防止対策として全数交換を行った同型のヒューズを確認したところ、本事象で破断したヒューズ以外にも４個のヒューズに損傷があったこと。また、メーカー推奨のヒューズの耐用年数を大幅に超えて使用しているものが多数あったこと。（２）ヒューズの検査の際に流用している「柱上変圧器取替時のマニュアル」について、ヒューズ自体を検査するように規定されていなかったこと。（３）電気係員に対する教育及び訓練について、令和２年１月に同マニュアルを用いた柱上変圧器の取替訓練を実施したものの、それ以降、同訓練を実施していなかったこと。よって、同種事象が多発しているなか、踏切障害事故を発生させる前に、確実にヒューズが破断した原因を追究するとともに、ヒューズの検査方法及び管理方法を見直すなど、適切に電気設備を維持管理すること。この際、ヒューズの交換の基準等を検討すること。この検討結果を踏まえ、同マニュアルの内容を見直し改正するとともに、電気係員に対して電気設備の検査等が適切に行えるよう必要な教育及び訓練を実施すること。２．電気設備の定期検査について、実施基準に従った取扱いが以下のとおり行われていない事実を確認した。（１）改正前の運転保安設備実施基準第７１条に規定する定期検査関係 ① 令和４年度の踏切保安装置の定期検査（検査周期３ヵ月）のうち一部の踏切保安装置について、検査基準日から起算した許容期間に至る前に検査を実施していたこと。 ② 令和４年度の信号装置の定期検査（検査周期３ヵ月）について、一部の信号機の検査結果が記録されていなかったこと。 ③ 令和元年度及び令和３年度の無線装置の定期検査（検査周期５年）について、検査結果が記録されていなかったこと。（２）電気設備実施基準第６７条に規定する配電線路の定期検査（検査周期３ヵ月）について、同実施基準第６７条別表第１０「電力設備検査方法」に規定する「避雷器の異常の有無」の検査項目が３ヶ月配電線路点検簿に記載されておらず、同項目の検査結果が記録されていなかったこと。よって、定期検査の許容期間内での実施及び検査結果の確認について、実施基準に従った取扱いが確実に行われるよう管理方法及び体制を改善すること。以上【四国運輸局】
