The construction company entered into subcontract agreements with Koei Kensou, which operates construction activities without the necessary permits under Article 3, Paragraph 1 of the Building Business Act, in multiple projects commissioned by Osaka City. In one of these projects, the company violated Article 22, Paragraph 1 of the same Act by having the main part of the contracted construction work undertaken collectively by Koei Kensou. Additionally, the company violated Article 24-8, Paragraphs 1 or 4 of the Building Business Act by failing to prepare or by falsifying construction management records or construction system diagrams in multiple projects commissioned by Osaka City.

The construction company assigned A as the chief engineer for a project ordered by Osaka City, in violation of Article 26, Paragraph 3 of the Building Business Act, by placing A at the construction site as a non-exclusive chief engineer when a dedicated engineer was required due to overlapping project schedules on other projects ordered by Osaka City. Additionally, the company violated Article 31, Paragraph 1 of the Building Business Act by submitting a forged false order form to the Osaka Prefectural Governor, which had the seal of another construction company that was not directly contracted for the project, falsely claiming that the company and the other construction company had re-established a direct contract.
