On May 14, 2019, Yamaguchi Construction Co., Ltd. entered as a primary subcontractor for a general prefectural road improvement project (River No. 1 Bridge P1) in Kaitsucho, Isahaya City, Nagasaki Prefecture. During work involving unloading scaffold components from a mobile crane's load platform at the company's material storage site in Onojima-cho, Isahaya City, an injured worker sustained a distal radius fracture requiring more than four days of leave, resulting in approximately two months of absence. The company was required to promptly submit a worker injury and illness report to the Isahaya Labor Standards Inspection Office but instead submitted the report on June 22, 2021, about 2 years and 1 month later. Consequently, the company and its representative director received summary orders imposing a fine of 200,000 yen each for violations of the Industrial Safety and Health Act, with the penalties finalized on January 5, 2022. This violation falls under Article 28, Paragraph 1, Item 3 of the Construction Business Act and is subject to administrative supervision.
