株式会社キトウ及び同社代表取締役は、令和３年１月16日、違反業者が請け負い施工する千曲市更埴字堂平地内の予防治山事業工事現場において、同工事に従事中の労働者が伐木の下敷きとなり、大腿骨骨折等の傷害を負い４日以上の休業に至ったのにかかわらず、同年11月18日に至るまで所管の労働基準監督署に労働者死傷病報告書を提出しなかった。 このことにより、株式会社キトウ及び同社代表取締役は労働安全衛生法違反により各々罰金20万円の略式命令を受け、その刑が確定した。 このことは、建設業法第28条第１項第３号に該当する。
