On November 19, 2019, during a main pipe replacement construction in Shibashima 3-chome, Higashiyodogawa-ku, Osaka City, a worker from Yoshikai Kogyo Co., Ltd., performing gas pipe removal work, was injured when a road curb fell, resulting in fractures of both malleoli of the right ankle. The company's then-director failed to promptly report the worker's injury to the Yodogawa Labor Standards Inspection Office as required, and did not submit the worker injury report until February 22, 2023. As a result, the former director was fined 100,000 yen for violating the Industrial Safety and Health Act (Law No. 57 of 1972), with the penalty confirmed on September 2, 2022. The company was also fined 100,000 yen on the same date. The Osaka Prefectural Government issued an instruction based on Article 28, Paragraph 1, Item 3 of the Construction Business Act, requiring the company to comply with laws related to construction work and to prevent similar incidents in the future.

令和元年11月19日、大阪市東淀川区柴島３丁目にて行われた、導管整備本支管入替工事において、本工事を施工する１次下請人である同社所属の労働者が、ガス管撤去に係る作業を行っていたところ、道路の縁石が落下し、右足関節両果骨折の負傷をしたが、当時の同社取締役は、淀川労働基準監督署長に対し、遅滞なく、労働者死傷病報告書を提出して報告しなければならないのに、これをせず、令和４年２月22日に至るまで、同署に労働者死傷病報告を提出しなかった。 このことで、同社の元取締役は、労働安全衛生法（昭和47年法律第57号）違反により、罰金10万円の刑に処せられ、令和４年９月２日にその刑が確定し、同社は、罰金10万円の刑に処せられ、同日にその刑が確定した。
