令和６年９月１２日から全国の鉄軌道事業者に対し指示した「鉄道車両における輪軸の緊急点検」の過程において、貴社及び株式会社総合車両製作所より株式会社総合車両製作所による作業記録の書き換えなどの不適切事案の報告があった。こうした作業記録の書き換えについては、輸送の安全確保の仕組みを根底から覆す行為であり、到底容認できるものではないことから、国土交通省において鉄道事業法に基づく保安監査を実施したところ、「１．確認された事実関係」に示す事実が明らかとなったことから、速やかに「２．講ずべき措置」に示す改善措置を講ずるよう指示する。１．確認された事実関係 (ア) 規程類に関する実態・輪軸の圧入作業を安全管理規程類（業務の受委託に関する事項）に基づかず、委託していた。・委託先において、圧入力値に関する規定や、規定された数値を逸脱した場合の取扱い等についての規程類がなく、事業者から図面を入手して圧入力値を確認したり、図面がない場合は自ら圧入力値を算出したりしていた（イ）現場における圧入作業の実態・委託先において、規定等から逸脱した輪軸をそのまま使用することが長く職場内で口頭で漫然と踏襲されていた。・委託先において、圧入力値の基準範囲を逸脱しても問題はないと認識していた。（ウ）係員の知識と教育の実態・委託先において、輪軸組立作業の知識に関する教育が体系的に行われていなかった。（エ）作業記録の書き換えの実態・委託先において、作業記録の書き換えが可能であり実際に書き換えていた。・委託先において、作業記録の書き換えは職場内で口頭で漫然と踏襲されていた。（オ）作業の管理の実態・管理的立場にいる者が、輪軸の使用の可否に係る判断に必要な確認を行っていなかった。以上の確認された事実関係は、鉄道に関する技術上の基準を定める省令第８７条第４項及び鉄道事業法第１８条の３第２項に抵触する。２．東急電鉄株式会社が講ずべき措置「２．確認された事実関係」を踏まえ、東急電鉄株式会社が講ずべき措置を以下に記載する。 (１) 規程類の整備・圧入作業に関する社内規程類を整備すること。・委託先の規程類ひいては実作業に自社の社内規程類が反映されるよう、適切に管理できる体制に改善すること。 (２) 教育体制の改善・委託先の圧入作業に関する教育及び訓練の管理ができるよう改善すること。 (３) 作業記録の書き換えの防止・委託先と協議のもと、作業記録の書き換えが容易に行われない仕組みを確立するとともに、貴社において必要な確認を行うこと。・委託先における内部監査等の仕組みを検証し、不適切な取扱いが見過ごされない体制を整備すること。 (４) 安全管理体制の点検と見直し・同様の問題が他の作業や部門で無いか点検し、必要な見直しを行うこと。３．報告期限２．（１）～（４）について、措置を講じ、又は、措置を講ずるための計画を策定し、２．（１）～（３）については、令和７年１月３１日までに、２．（４）については、令和７年３月３１日までに報告すること。【鉄道局】
