令和３年２月１３日、佐藤建設(株)の労働者が、福島県南相馬市小高区金谷字上新田地内の現場において「張ブロック撤去作業」に従事していた際、ワイヤーロープのフックが外れ、傍らにいた同労働者の右腕に直撃したことによって、休業４日以上を要する労働災害が発生し、同労働者は同日から休業した。 令和３年３月２日、佐藤建設(株）の代表取締役及び同社の課長は、共謀の上、当該労働災害を「佐藤建設（株）が直接請け負った、福島県南相馬市小高区田町地内の解体工事現場で発生した事故」と偽って、相馬労働基準監督署に労働者死傷病報告を提出し、もって労働基準監督署長に虚偽の報告を行った。 このため、労働安全衛生法に違反したとして、令和５年１２月１５日付けで佐藤建設(株)、同社の代表取締役及び同社の課長に対し、それぞれ２０万円の罰金刑が確定した。
