Safety violation or accident勝田環境株式会社は,令和3年5月24日,本市発注の都市下水路及び排水路清掃業務委託にお いて,雨水管を高圧洗浄車にて洗浄作業中,下流人孔から挿入した洗浄ホースが上流人孔に到達し た際,洗浄ホースの向きを確認し,合図者の合図を確認して洗浄ホースの水圧をかけなければなら ないところ,合図を確認しないまま圧力をかけ,その圧力により下流側人孔に勢いよく洗浄ホース が飛び出し,下流側人孔で作業をしていた作業員の顔に当たり,顔面を負傷する事故を起こした。 水戸市建設工事及び委託業務の契約事務に関する規程(以下「契約規程」という。)第75 条第1項別表第3第6項「安全管理措置の不適切により生じた工事関係者事故(市発注工事 等)」に該当する。 「契約規程」 措置要件期間 別表第3第6項ア 当該認定をした日から 「安全管理の措置の不適切により工事関係者を負傷させたと認 2週間 められるとき。」
