The safety audit of Matsuura Railway Co., Ltd. was conducted from June 20 to June 22, 2022. As a result, several issues requiring corrective action were identified. The company was instructed to investigate the root causes, implement measures to prevent recurrence, and report on these measures by October 12, 2022. Specific issues include: 1) some track signage at Kurayado Station and between Yoshi Station and Kanda Station was not clearly visible; the company must verify signage visibility across the entire line, take necessary corrective actions, and regularly train staff on track standards. 2) Inspection records for level crossing safety equipment from FY2019 and FY2020 were incomplete, particularly the measurement records of warning times for crossing signals, which are required during regular inspections to ensure compliance. The company must identify all locations needing measurement, perform measurements, verify compliance, and properly document inspections of electrical equipment, including new, modified, or repaired installations, ensuring records are maintained and deterioration is monitored. 3) Signal flare management for train protection lacked records of receipt dates and exceeded manufacturer expiry dates. The company must review management practices, update procedures to track expiry dates accurately, and revise relevant guidelines in accordance with legal procedures. The Ministry of Land, Infrastructure, Transport and Tourism (Kyushu Transport Bureau) issued these instructions to ensure safety and compliance.
