Safety violation or accident水戸市環境整備事業協同組合は、本市発注の水戸地区燃えるごみ及び燃えないごみ収集運搬 業務委託において、令和8年3月17日、塵芥収集車(運転手と作業員が2名で乗車)で運搬 した廃棄物を焼却施設のごみピット内に投入し、同施設を出た後、荷箱内に異物が残っていた ことから、車両を一時停止させ、作業員が異物の確認作業を行った。その際、車両とテールゲ ートに挟まれる事故を防止するための安全棒を作業員が設置せずに作業を行ったこととあわ せ、作業員の作業状況を確認せずに運転手がテールゲートを下げてしまうなど、不適切な安全 管理の措置により、作業員がテールゲートに挟まれ、死亡する事故を生じさせた。 水戸市建設工事及び委託業務の契約事務に関する規程(以下「契約規程」という。)第75条 第1項別表第3第6号「安全管理措置の不適切により生じた工事関係者事故(市発注工事等) 」 のウに該当する。 「契約規程」 措置要件期間 別表第3第6号 当該認定をした日から ウ安全管理の措置の不適切により工事関係者に死亡者を生 2カ月 じさせたと認められるとき。
