The construction contractor in Osaka engaged Ishinaka Corporation to operate a mobile crane at a private construction site. Due to improper construction management as the main contractor, the crane lost balance and tilted, causing its arm to contact and break the suspended steel beams. The falling beams damaged the roofs of two nearby houses, causing public hazards. The violations include: (1) Not establishing work methods, overturn prevention measures, worker placement, and command systems considering site size, terrain, geological conditions, and load weight; (2) Not preparing a work plan indicating safe work methods for steel assembly on buildings over five meters high; (3) Using a mobile crane without installing necessary ground reinforcement such as steel plates with adequate size and strength in areas with soft ground, thereby risking overturn; (4) Not informing the crane operator of necessary safety measures, including installing steel plates considering ground softness, to prevent machinery-related occupational accidents.
