Between November 8 and 11, 2021, a safety audit was conducted on Watraskeikoku Railway Co., Ltd., which revealed several compliance issues. The company was instructed to take corrective measures and report by February 14, 2022. The identified issues included: (1) failure to inspect rail wear on all straight and some curved sections (excluding tunnels) as per Railway Construction Standards; (2) failure to inspect vehicle roofs during important parts and general inspections; (3) failure to notify train operation plan changes following schedule adjustments in March 2019, as required by the Railway Business Act; (4) errors in driver qualification management reports submitted to authorities; and (5) inadequate alcohol breathalyzer checks for train crew, including lack of post-shift checks, improper recording of alcohol levels, and non-compliance with internal regulations and standards. The company was ordered to investigate root causes, implement preventive measures, revise internal procedures, and establish systems to ensure proper alcohol testing and reporting in accordance with the Railway Operation Handling Standards and related regulations.
