（株）角翔は、解体工事業を営む事業者であるが、同社の上位会社から請け負った「令和５年度中間貯蔵家屋等解体工事（その２）」において、令和６年８月８日、同社の労働者が当該作業件名の現場内で作業中に、同僚の操作する重機が掴んでいた鉄骨が右足に激突し、右足関節打撲及び右足打撲の負傷をしたために４日以上の休業を要した労働災害について、本件現場を所轄する富岡労働基準監督署に対し、遅滞なく労働者死病報告を提出しなければならないにもかかわらず、偽りの被災場所と被災理由を記載した労働者死病報告を郡山労働基準監督署に対し提出し、本件現場を所轄する富岡労働基準監督署に対し、令和６年８月３０日に至るまで、真実が記載された労働者死病報告を提出せず、もって法令に定める報告を行わなかった。 このため、労働安全衛生法に違反したとして、令和７年７月１２日付けで（株）角翔及び同社専務取締役に対し、それぞれ２０万円の罰金刑が確定した。
