| Original Japanese text | 令和４年１０月１１日から１０月１４日まで、貴社に対して保安監査を実施したところであるが、保安監査の結果、改善を要する事項が認められたことから、下記の事項について、改善措置を講ずるよう指示する。 講じた措置については、令和５年４月１７日までに報告されたい。 なお、改善措置を講ずるにあたっては、背後要因を含め当該事象が発生した原因を究明したうえで、再発防止に必要な改善策を策定するとともに、輸送の安全に係る業務が確実に実施できるよう留意すること。 記 １．軌道の検査について、以下の事実を確認した。 （１）線路設備実施基準第３８条に規定する本線の静的軌道変位検査について、同実施基準第２７条に規定する高低の整備基準値を超過している箇所があったにもかかわらず、整備をしていなかったこと。 （２）線路設備実施基準第３８条に規定する分岐器の静的軌道変位検査について、同実施基準第２８条に規定するクロッシング部の軌間の整備基準値を超過している箇所が複数あったにもかかわらず、整備をしていなかったこと。 整備基準値を超過している箇所については、既に整備が完了したと報告を受けたところであるが、今後の鉄道施設の維持管理にあたっては、実施基準等に基づく保守管理が適切に実施されるよう、社員教育の見直しを行うとともに、整備実施状況に係る社内チェック体制の強化を図る等、管理方法及び体制を見直すこと。 ２．線路設備実施基準第４２条に規定するレール締結装置の検査について、踏切道に敷設されている敷板下のレール締結装置の検査内容が定められていないため、レール締結装置の検査の判定が正確にできておらず、広域農道踏切道において複数の犬くぎの腐食及び犬くぎ頭部の摩耗により締結状態が不良であることを確認した。 当該踏切道の不良箇所については、既に整備が完了したと報告を受けたところであるが、今後の鉄道施設の維持管理にあたっては、実施基準等に基づく保守管理が適切に実施されるよう、踏切道に敷設されている敷板下のレール締結装置の検査内容を規定し関係者に教育を行うとともに、検査及び整備実施状況に係る社内チェック体制の強化を図る等、管理方法及び体制を見直すこと。 ３．土木設備実施基準第８条に規定する建築限界について、津軽五所川原駅のプラットホームにおいて支障している箇所があることを確認した。 よって、同実施基準に基づきプラットホームが建築限界を支障しないよう、必要な措置を速やかに講ずること。 また、今後の鉄道施設の維持管理にあたっては、実施基準等に基づく保守管理が適切に実施されるよう、社員教育の見直しを行うとともに、検査及び整備実施状況に係る社内チェック体制の強化を図る等、管理方法及び体制を見直すこと。 ４．運転保安設備実施基準第２４条第１項において準拠される表－２に規定する保安通信設備の検査について、木柱、腕木及び支線の腐食並びに碍子の脱落があり、同実施基準第２８条に規定する検査の結果に応じた処置が適切に行われていなかったことを確認した。また、当該設備の修繕が計画的に行われておらず、さらに、施設管理者による修繕計画の進捗管理ができていなかったことを確認した。 よって、同実施基準に基づき必要な措置を速やかに講ずること。 また、今後の鉄道施設の維持管理にあたっては、実施基準等に基づく保守管理が適切に実施されるよう、法令等の遵守を含め関係者に徹底するとともに、修繕計画及び進捗状況等の情報共有を適切に行い、施設管理者が保安通信設備に係る保守管理の現状を適確に把握できる仕組みを構築する等、管理方法及び体制を見直すこと。 ５．車両整備実施基準第１０条に規定する無線装置及び運転保安設備実施基準第２４条第１項において準拠される表－２に規定する沿線電話の検査を実施しておらず、また、施設部門と車両部門との間で無線装置及び沿線電話の検査の実施状況が共有されていなかったため、令和３年６月１日から令和４年１０月６日まで間、無線装置及び沿線電話のいずれも使用できない状態のまま動力車を操縦する係員が単独で乗務する列車を運行していたことを確認した。 既に、無線装置及び沿線電話を使用できるように措置したとの報告を受けているが、今後の鉄道施設の維持管理にあたっては、各実施基準の教育を実施するとともに、検査を確実に実施すること、加えて列車の運行に関する事項を適宜社内において情報共有を行う仕組みを構築する等、部門間の連携も含めて管理方法及び体制を見直すこと。 ６．実施基準管理規定第１１条に規定する機関士の適性検査（身体機能検査）について、視力の検査は行っていたものの、色覚、視野及び両眼視機能の検査は行っていなかったことを確認した。 機関士に対するこれらの検査については、既に検査が完了したと報告を受けたところであるが、今後は、機関士の適性検査（身体機能検査）について、適切な実施と確実な判断が行えるよう、管理方法及び体制を見直すこと。 【東北運輸局】 |
