Ikezawa Juki, on February 21, 2020, at a demolition site located in Yodogawa Ward, Osaka City, conducted demolition work involving steel structures over 5 meters in height. They did not appoint a construction supervisor who had completed the skill training required by law for steel structure assembly work. During the work, a worker used a gas cutting machine to cut steel, and sparks from the cutting ignited building materials, causing a fire. The worker sustained burns and died two months later. As a result, the company was charged with violations of Article 14 of the Industrial Safety and Health Act, Article 6 of the Enforcement Ordinance of the same Act, and Article 517-4 of the Industrial Safety and Health Regulations. A summary order imposing a fine of 200,000 yen was issued by Osaka Summary Court on June 18, 2021, and finalized on that date. This is recognized as falling under Article 28, Paragraph 1, Item 3 of the Construction Business Act.
