The executive director of Yūsei Giken Ltd. is responsible for worker safety management when the representative director is absent. On August 11, 2020, at the company's processing plant located at 23-1 Machibata, Shimokawasaki, Nihonmatsu City, Fukushima Prefecture, the company allowed workers to organize stacked formwork materials using a forklift, a vehicle-type lifting and transportation machine. The executive director operated the forklift and allowed a worker to ride on the fork outside the driver's seat, and to perform the task of organizing the stacked formwork materials. The forklift was used in a manner outside its primary purpose, and necessary measures to prevent danger from machinery, tools, or equipment were not taken. As a result of this accident, the worker fell from the forklift when it tipped over during travel, suffering a burst fracture of the first lumbar vertebra and conus medullaris injury. The company was fined 200,000 yen, and the executive director was fined 500,000 yen.
