有限会社城下建設は、県央広域本部上益城地域振興局が発注した「上益城管内治山事業（交付金）林地荒廃防止事業火山第1号工事」の元請として同工事を行っていたものであるが、同社代表取締役は、同社取締役と共謀の上、同社の業務に関し、令和2年（2020年）8月7日、同工事現場において、同社の労働者が樹木の運搬作業中に運搬していた樹木が衝突して骨折の傷害を負い、4日以上休業することになったにもかかわらず、労働者死傷病報告書を遅滞なく熊本労働基準監督署長に提出しなかったものである。 この件に関し、同社に対し、熊本簡易裁判所から労働安全衛生法違反により罰金20万円の略式命令があり、令和4年（2022年）2月3日、その刑が確定した。
