令和５年９月４日付けで法第65条第２項に基づく監督処分（令和５年９月25日から令和５年10月９日までの15日間の業務停止）を受けたが、この処分に違反して、当該期間中にインターネット上に広告を掲載していた（法第66条第１項第９号後段該当）。 上記の事実につき、令和５年９月29日付けで法第72条第１項に基づく報告命令に従わなかった（法第72条第１項違反）。上記の業務停止命令処分は、計４回にわたり、法72条第１項に基づく報告命令に従わなかったことが、その原因の事実であり、さらに同様の違反を繰り返したことは、特に情状が重い（法第66条第１項第９号前段該当）。
